The Heart Foundation offers a peer support program called Supporting Young Hearts.

Young people aged 18-40 years living with a heart condition or recovering from heart surgery have unique needs and face enormous challenges associated with living with a chronic illness at a critical life stage.
In 2015, in response to demand from young people searching for information and a coordinated manner to connect with peers, the Heart Foundation in Victoria developed ‘Supporting Young Hearts’ (SYH).
SYH aims to help younger people connect, support each other by sharing stories and learning about how to manage their condition and keep their hearts healthy.
SYH members tell us it is like a special family; helping them out of a darkness they could not otherwise see through.
