ABOUT US
The Chronic Disease Self-Management Special Interest Group was formed in February, 2004.
Its membership is health professionals and consumers in Victoria who wish to promote self-management programs as means to improve the health and quality of life of all people with chronic illnesses. Membership is open to all health professionals and consumers who would like to share their work, or learn more, in this area.
Many of the members of the Special Interest Group run self-management programs as part of the broader programs they developed under the Hospitals Admissions Risk Program (HARP), Integrated Disease Management (IDM) programs and more recently under the Australian Better Health Initiatives.
The group meets every second month. We have presentations about new programs and recent research, we discuss programs, how to evaluate them and share resources.
Self-management programs can now be found in hospitals, community health services and consumer support groups across Victoria. As this site grows many of them will be advertised here but for more information about where to find self-management programs please visit the Service Directory on the Better Health Channel website www.betterhealth.vic.gov.au
